The

program will take place on Friday, August 25, 2000 from

9:30a.m. to 2:30p.m. at the Bloom Township Center, 425

So. Halsted Street, Chicago Heights, IL.

Free identification cards

are available for persons 65 years of age and older and

persons with disabilities. Applicants are required to

prove name, date of birth, Illinois residency and prove

signature for comparison.

The State ID is exactly

like a drivers license in appearance, but used only for

identification purposes only. If you don't drive, a

picture Id is usually required at stores for check

writing approval, to obtain senior discounts at stores or

restaurants, and as a certified secondary form of ID in

applying for governmental entity programs like

transportation or public aid.

Applicants are asked to

bring in a social security card, check or savings book,

drivers license, current utility bill, baptismal or

marriage certificate as proof of identification.

If you want to reserve

your space for the State ID card, or need additional

information, please call the Senior Department at

708-754-9400 ext 244.
